Referrals and Enrollment Our goal is to have clients/families become more self-sufficient. Individuals can enroll or be referred to CWAH for basic needs services for a one time visit. These services include: Food Pantry, Clothing Boutique, Toiletries, Household Items, and Furnishings and Appliances. A scheduled appointment is required to complete an application with our Family Coordinator. We accept referrals from: - Social Services Agencies, Religious and Educational Institutions, and Human Services Agencies (case workers, social workers, pastors, counselors, etc.)
- Authorized personnel making the referral who are informed about the individual's living situation and reasons for basic need services
- Referrals may also be made over the phone, via CWAH Staff or Board Member
In order to receive services, Clients are required to: - Engage in creating an active plan to become more self-sufficient
- Consistently participate in relevant programs and/or workshops
- Communicate any changes in status to the Family Coordinator
*Please note that certain items such as furniture, appliance, etc. may not be available to a new enrollee because the participating CWAH clients take precedence for those items. Should no actively enrolled client need those particular items, a new referral may be able to receive those particular items.
